• To add or remove dependents
• To notify of any changes affecting your insurance coverage
• Failure to notify benefits updates in a timely manner could result in loss of coverage for new dependents or, in the case of divorce, could result in the employee having to pay back premiums paid on behalf of an ex-spouse

Eligibility Documentation

For all new dependents, records such as birth certificates, marriage licenses or a Domestic Partner Affidavit are required to be submitted to Benefits at [email protected].

Changes to Personal Information

It is the employee’s responsibility to make sure Centric has the correct personal information. This will ensure that employees have up-to-date, complete coverage in the various benefits plans and receive communications in a timely manner.
