Centric partners with Prudential to offer Basic Term Life and Accidental Death & Dismemberment (AD&D) coverages.
Employees may purchase additional supplemental coverage to increase your coverage amounts for yourself and dependents.
Voluntary Life and AD&D Insurance
Employees may purchase supplemental Life & AD&D up to 5x their annual base salary up to $500,000. Amounts exceeding the lesser of 2x your salary or $300,000 will require evidence of insurability.
Voluntary Spouse and Dependent Life and AD&D Insurance
If you elect voluntary life coverage for yourself, you can also elect voluntary life coverage for your spouse/domestic partner in increments of $10,000 up to a maximum of $250,000 or 50% of the employee approved amount. Dependent child life coverage is offered at $10,000. Spousal life exceeding $30,000 will require evidence of insurability. Employees that have waived coverage for their spouse previously will need to complete Evident of Insurability for any amount requested.
What Is Evidence of Insurability (EOI)?
EOI or Statement of Health is information about your health, which the insurance company may require to approve you for coverage.
If you’re newly eligible and have not previously waived coverage, you can elect up to the guaranteed issue amount without submitting EOI. You may be required to submit EOI if you have previously waived this coverage or if you elect an amount above the guaranteed issue amount.
