Preventive Visit Incentive

All full-time, part-time and W2 employees that receive a preventive exam and turn in an EOB (Estimate of Benefits) to [email protected] anytime between November 1, 2025—October 31, 2026 will be entered into a prize drawing later this year! You do NOT need to be enrolled on the Anthem Medical plan to participate.
